New Client Forms
For your convenience, we have made a number of forms available for new clients to download. We encourage you to read over the Client Handbook and print out the remainder of the new client forms (Client Information Form, Agreement to Pay for Professional Services Form, Consent for Treatment, Release of Info). Filling these out in advance of your first visit will minimize the time you spend in the waiting room completing them upon your initial appointment.
Methods of Payment
The Next Step Counseling Services, LLC accepts Tricare, Humana, Anthem, Cigna, Medicaid and most private insurances. We also accept private pay referrals. Additionally, our office accepts cash, check and credit card payments. Fees are due at the time of service.
Our office requires a 24 hour notice in order to cancel an appointment. All appointments cancelled or not kept without a 24 hour notice will incur the full session charge. On the rare occasion that a missed appointment is due to an emergency, a minimum of a $25.00 fee will be assessed.
